our process

Our process is simple and does not require a visit or referral from a physician. Our one-on-one exercise services are provided in-home, whether you are at a private residence or a senior living facility, we come to you. 

+ We begin with an in-home consultation at no cost to you or your family. We want to understand if we are going to be a good fit for you and your family. At this visit we will also discuss goals, expectations and wishes moving forward. 




+ Following the initial consultation we complete our extensive assessment process which includes physical and psychological measurements to better develop the exercise programming. During the assessment process we will utilize state of the art technology (Force Decks & HumanTrak) from VALD Health to measure strength, balance, mobility and postural stability. 




+ After the assessment our team of Wellness Specialists develop a plan of care and an exercise program to fit your needs. You and your family will determine a regular schedule in which your Wellness Specialist will come to your home in order to lead the one-on-one exercise program. Many of the members within LiveWell Health choose to exercise two or three times under the supervision of a Wellness Specialist.

Physical therapy is often provided through insurance companies therefore a physician’s order is needed. Medicare also calls for strict guidelines to be followed. 

Our private pay model allows for great flexibility when designing and implementing a program.

LiveWell does not accept Medicare or Medicaid. However, our services may be covered under a long-term care insurance policy. The best option going forward is to understand the details and benefits of your individual policy, we recommend calling your agent or the insurance provider to understand more.
