* Strength, stability, balance, and mobility assessments
* Cardiovascular and fitness assessments, including VO₂ max testing
* Personalized exercise and wellness programming
* One-on-one sessions with LiveWell Wellness Specialists
* Post-hospitalization and recovery-focused wellness support
* Fall prevention and balance strategies
* Longevity coaching and health optimization
* Digital coaching and wellness platforms
* Progress tracking and objective health measurements
* Educational materials, articles, newsletters, and other health and wellness content
Services may be delivered in person, virtually, through digital platforms, or through a combination of methods.
## 2. Wellness Services Are Not Medical Care
LiveWell Health provides wellness, exercise, fitness, and health optimization services. **LiveWell is not a medical practice, and our Services are not intended to diagnose, treat, cure, or prevent any disease or medical condition.**
Our Services do not replace medical care, physical therapy, occupational therapy, nursing care, or other services provided by licensed healthcare professionals.
Any assessments, measurements, recommendations, exercise programs, educational materials, or other information provided by LiveWell are intended to support wellness and functional improvement and should not be interpreted as medical advice or a medical diagnosis.
You should continue to consult with your physician and other qualified healthcare professionals regarding medical conditions, medications, symptoms, treatment decisions, and other healthcare needs.

## 3. Individual Results May Vary
LiveWell uses objective measurements and individualized programming to help members understand their current abilities and track progress. However, individual results vary.
Factors including age, health history, medical conditions, medications, consistency, lifestyle, nutrition, sleep, genetics, and other circumstances may influence outcomes.
LiveWell does not guarantee any particular health, fitness, functional, longevity, or performance outcome.
## 4. Assessments and Recommendations
LiveWell assessments are designed to provide information about an individual's current functional abilities, fitness, and wellness.
Assessments may include measurements related to strength, balance, mobility, cardiovascular fitness, gait, functional movement, body composition, or other health and performance metrics.
Assessment results are intended to help inform personalized wellness programming. They are not intended to diagnose a medical condition or replace evaluation by a licensed healthcare professional.
You are responsible for providing LiveWell with accurate and complete information regarding your health history, medications, limitations, injuries, medical conditions, and other information relevant to your participation.
## 5. Participation and Personal Responsibility
You acknowledge that participation in exercise and physical activity involves inherent risks, including but not limited to muscle soreness, strains, sprains, falls, cardiovascular events, injury, and other adverse outcomes.
You are responsible for communicating honestly with LiveWell regarding your current abilities, symptoms, health history, and any changes in your health.
You should not participate in any activity that you believe may be unsafe for you. LiveWell may recommend that you seek medical clearance or consult with your healthcare provider before beginning or continuing certain activities.
## 6. Wellness Specialists
LiveWell Wellness Specialists provide individualized exercise and wellness coaching based on each member's assessment, goals, abilities, and needs.
Wellness Specialists are not physicians and do not provide medical diagnoses or medical treatment unless separately licensed and acting within the scope of an applicable professional license.
LiveWell may match members with Wellness Specialists based on factors including goals, location, availability, experience, training, and individual needs. LiveWell reserves the right to modify or change a member's Wellness Specialist when reasonably necessary to maintain service quality and continuity.
## 7. Assessment Specialists and Wellness Directors
LiveWell may use Assessment Specialists to conduct initial assessments and collect information used to develop an individual's wellness plan.
LiveWell may also use Wellness Directors or other supervisory personnel to oversee programming, quality, member experience, and the coordination of services.
These roles are part of LiveWell's wellness model and do not constitute medical diagnosis or treatment.
## 8. Physician and Healthcare Provider Collaboration
With appropriate authorization, LiveWell may communicate with a member's physicians or other healthcare providers to support continuity of care and appropriate wellness programming.
LiveWell does not control the recommendations, treatment decisions, or services provided by independent healthcare professionals.
Nothing on the LiveWell website or through our Services should be interpreted as establishing a physician-patient relationship between you and LiveWell.
## 9. Digital Platforms and Third-Party Services
LiveWell may use third-party technology providers to support scheduling, communications, payments, digital coaching, exercise programming, data collection, documentation, or other aspects of our Services.
Your use of certain LiveWell Services may therefore also be subject to the terms, conditions, and privacy policies of those third-party providers.
LiveWell does not guarantee the uninterrupted availability, security, or functionality of third-party platforms and is not responsible for failures or interruptions caused by third-party systems.
## 10. Longevity Coaching Platform
The LiveWell Longevity Coaching Platform may provide personalized exercise recommendations, educational resources, progress tracking, assessments, and other wellness tools.
The platform is intended to complement—not replace—professional medical care.
Information provided through the platform is not a diagnosis, medical treatment plan, or substitute for consultation with your physician or other qualified healthcare professional.
## 11. Fees, Payments, and Scheduling
Certain LiveWell Services require payment.
Prices, payment terms, scheduling policies, cancellation policies, and other applicable service terms may be communicated to you before or during enrollment and may be incorporated into your agreement with LiveWell.
Unless otherwise stated in writing, use of LiveWell Services does not create a guaranteed minimum service period or long-term commitment.
LiveWell reserves the right to change pricing for future Services with reasonable notice.
## 12. Cancellations and Missed Appointments
Members are expected to provide reasonable notice when canceling or rescheduling appointments.
Specific cancellation and missed-appointment policies may vary by service and will be communicated to members at the time of enrollment or scheduling.
LiveWell reserves the right to charge applicable fees for late cancellations or missed appointments in accordance with the applicable service policy.
